Please complete the below paperwork and return to school on or before the first day of school.  Some forms are by family, others by student.  Completed forms may be emailed to sttherese@stthereseschoolmunhall.org or dropped off at supply pick up/drop off.  Thank you!

Catholic Schools Parent Memorandum of Understanding document for review, and the Memorandum of Understanding form to print and sign.

 

Emergency Forms will be distributed during supply drop off/pick up.   Please return those forms to the office on the first day of school.
